Creating Request Forms

You've successfully configured the workflow automation (Notification Templates and Approval Flows). Now, you need the tool that you will actually use to start the process: the Request Forms.

Request Forms are the essential templates used by employees to submit any request. By creating the forms, you ensure all necessary information is collected upfront and automatically link the submission to the correct Approval Flow you've already defined.

Note: Only users with admin access can create and manage request forms.

Creating Request Forms


To create a Request Form, follow the steps:

Go to WE → Requests → Request Forms

Click the Create Request Form button

STEP 1. General Details

  • Request Form Title: Enter the mandatory name of the form (e.g., Equipment Purchase Request, Training & Development Application).
  • Type Description: You can add a short description to explain the form’s purpose or what information should be included.
  • Icon: You can select an icon to make the form easy to recognize in the Request Forms list.
  • Color: You can choose a color to visually differentiate this request type from others in the main list.
  • Request Items: Click the Add button to include items and create the necessary input fields.
    • In the Add Items sidebar, configure the following fields: 
      • Enable Divider: You can enable this switch to insert a horizontal line in the form. Use it to group related request fields and improve readability.
      • Title: Enter a name or question for the field (e.g., “Requested Item Name,” “Start Date”).
      • Type: Use this dropdown to define the kind of input you will provide (e.g., Text Area, Radio Buttons, Instruction, etc.).
      • Required: You can enable this switch to make the field mandatory. You will not be able to submit the request until you fill it out.



After configuring your items, click + Add New Item to add more fields, and then click Save when the form is complete. Don’t forget to click Save and Continue to proceed to the next step.
STEP 2. Configurations

After defining the content of the form in Step 1, this step links the form to your pre-defined Approval Flows and Notification Templates.


  • Request Approvals
    • Approval Required: Enable this switch if the request must pass through an approval process before being finalized. If OFF, the request is instantly created/finalized upon submission.
    • Approval Flow: Choose the specific Approval Flow (sequence of approvers) that this form should follow. This is a list of the flows you previously created in Requests → Settings.


  • Notifications
    • Send Notification: Enable this switch to trigger system and email notifications when the request is approved or rejected.
    • Select Template:
      • Approve Template: Choose the Notification Template that will be used to notify the requester when the request is approved.
      • Reject Template: Choose the Notification Template that will be used to notify the requester when the request is rejected.

STEP 3. Preview and Add

The final step allows you to review the completed form structure, see how it will appear to the user, and finally click Save and Add to activate the new Request Form. This preview is available under the Overview tab.

You can also see the Employees and Documents tabs for Step 3:

Employees: assign the employees who will be able to use the request.

Documents: Upload any important reference documents that employees may need to see when using the form. (e.g., policies, contracts, or guidelines).

Once you have a list of Request Forms, displayed as cards, you can search and find the one you need instantly using the Search bar and Filter options (Active, Archived, Draft). You can also copy, delete, or preview forms by selecting the corresponding options from the Actions menu.
